for totally smooth i had to disable all vsync options in the nvidia-settings, i alsa had to disable flipping.
then install xbmc-standalone and login with XBMC as my session type instead of gnome.
a minimal install seems better at the moment.

in a terminal type:
sudo apt-get install xbmc-standalone
this installs it so when you login xbmc is loaded instead othe desktop (you can change it between xbmc and the desktop at each login.
now type:
nvidia-settings
and on the left hand side select "OPENGL SETTINGS" then untick vsync
then click on OPENGL/GLX and untick vsync and flipping.
now open xbmc and under settings->system->video make sure vsync is set to "always enabled"
then exit xbmc. log out and when you log in (click on your username, but dont type password or click ok) at the bottom of the screen you can change "session type" to XBMC instead of gnome.
You should have a nicely working xbmc now.
if you want to go back to the desktop press "s" in xbmc and pick exit xbmc, then when you log in switch the "session type" back to GNOME.
it worked for me, hopefully it'll work for you. But as with all computers, there's no guarantees.

sorry, if you're running xbmc live i dont think this will help.

Hope if there also a solution for the xbmc non-standalone version, because if i log in xbmc-standalone i lost the possibility to download with jdownloader or torrent. sorry for english.

i am amazed at running xbmc as standalone in its own session.
no dropped frames in movies that usually drops about hundred.
25fps 1080 movies always worked great in desktop but not 23.976.
what can be the problem with running xbmc in gnome (lucid 10.04)?

Same HERE, only on 23.976 movies. early i've made a few test...one of those is to disable stuffs/service that gnome runs at boot, and MAYBE i've found the trick. i've disabled the clock applet either the icon to restart/shutdown the session (all of the applets in the right up corner) and it seems that the stutter every 1 minute is gone, or maybe it's just me that i'm going sick. anyone can test it? bye

yea, had the same issue. gone to a minimal install of karmic. and that's ok so far (except for one issue - http://forum.xbmc.org/showthread.php?tid=73860)

Just for the record:
It worked on an
ASRock ION 330 HT with 10.04 Lucid Lynx and a Panasonic Viera ΤX-P46G10
using:
Modes "1920x1080_60_0" "1920x1080_24"
The only modification I made was in the command of step: 4. Adjusting the settings in NVIDIA's drivers.
Instead of
..."GPUScaling[DFP-1]=1,1"...
I used
..."GPUScaling[DFP-0]=1,1"...

there should be one additional info regarding the script:
as of now, it only works when logged in as "xbmc" user, and the "xbmc" user absolutely needs to be in the superusers group. which is a shame, because all changes to the system I do as a root user via SSH, never as the xbmc user itself (which is also never in the superusers group, the whole internet knows the password of an "xbmc" user
