None of my TV shows are rared (TV Shows\Showname\showepname.mkv) I tired adding a new show, and also removing, and rescanning a show to the TV database. No luck.
I don't know if you've already found the answer to the missing logos, but i,ve been having the same problem with "logo missing" in XBMC. All my posters and fanart and episode images were okay, but the logo's, landscape and clearart would not get displayed.
My Log file also showed something like:

It seems that you need create credentials for your SMB Server/Share.
I did this by adding a new source, then it asked me for username and password. Then i checked "Remember this..." or somthing like that.
After that all my logo's started showing up again.
After that i saw that the credentials are stored in the passwords.xml file, which is located in:
[Windows 7] "\Users\Username\Appdata\roaming\xbmc\userdata"
[Ubuntu ] "/home/username/.xbmc/userdata"
Try it out, hope this fixes your problems.
