Harmony. Definitely Harmony IMHO.
It does what you ask and together with EventGhost it can automate so much of your PC...
For troubleshooting and bug reporting please make sure you
read this first (usually it's enough to follow instructions in the second post).

I was about to post exactly the same question.
I intend to buy an iPad3 when they're released (I don't currently have an iPad, there seems little point in buying one when the new one is imminent an besides, I've not found the solution to my problem yet) and I want to use this.
My current plan is to buy a Pioeer SC-LX85 AV amp (partly due to the wonderful iPad app) so will want to control this, along with my TV, MacMini running XBMC etc.
Anyone had any success with the Harmony Link software (I think it's currently only available in the US)?

I'm looking for a solution which will allow me to create activities such as watch film - turn on AV receiver, tv, change tv input etc. Not sure these will allow that sort of thing?

You can easily set up "macros" that execute a series of commands that will do what you want. Macros can be set up on the device selection keys that will switch inputs and outputs to whatever is needed. There is a bit of a learning curve but it's not too difficult and the "experts" in the forums go out of their way to help.
