I solved this issue, but it wasn't easy, so I'm sharing my insight.
Scraping did NOT use the embedded / tagged covers for many of my songs. Instead it found inferior covers somewhere (even when I selected update all in the files section). I turned off all options in the Universal Album Scraper, and I turned off "prefer online" in the Music settings screen. I also removed the music library before scraping and ensured there were no random covers in my Artist Information Folder. Yet bad covers kept popping up (occasionally).
The answer was to delete the thumbs folder.
