Could be a permissions issue.
Try SSHing into the iPad and running this command:

You didn't turn on the debug setting in xbmc -> settings -> system so i can only make the assumption that this file is 1080p resolution?
Whatever it is - the hardware decoder doesn't like the encoder settings used to generate that file. If its 1080p its encoded out of spec with 9 ref frames.

In the paste above is the xbmc.log with debug mode turned on, or have I understood something wrong?

your xbmc.log file is good, we just want you to tell us what
MediaInfo (wiki) says about the file. It gives more details about the codecs in the file and all that stuff. That way we can see what settings where used to create the file and see if that's the reason it's not working.
Basically, we just need a little more info.

Hi10P. That's a profile of h.264 that a lot of anime groups like to use now, despite it being non-standard in the consumer space, and no hardware decoders supporting it. Anything using Hi10P has to use software (CPU) decoding, and the ATV2/iOS won't be able to handle it. It's a widespread problem for lots of HTPCs, even ones using Intel ATOM processors. Anyone who needs hardware decoding is screwed with these files. However, you can normally find a group/version that uses "8-bit" aka, normal High Profile for h.264. That's what I've been doing.
