I'd just like to report something that I've found today. My apologies if it's been noted before. I had several of my tv shows scan and were duplicated. I did a fresh install of xbmc and it picked up several shows with folder art, posters , etc but no episodes were displayed at all. The only way I could fix this was to go into file mode and select tv information and tell it to ignore local stored info and fetch new info. Then they appeared normally. I had used Ember Media Manager in the past to setup custom fanart etc. It appears that xbmc didn't play nicely with the local stored data from my tv shows.

It's because the nfo files contain no episodeguide - the Debug Log will tell you this during scrape. i.e. the nfo files are invalid.
