If anyone comes across this thread that has the same issue, I can confirm it was the receivers fault. I just switched to the Pioneer VSX-1122 and both HD audio codecs are bitstreaming properly. Not quite sure what the problem was, especially when the VSX91TXH supports TrueHD / DTSMA. Anyway, thanks to all who tried helping.
