What is the source of your channels? DVB, ATSC, QAM, or old school analog? Analog tuners typically mark all recordings as 4:3, even if you've configured a set top box to feed it 16:9 analog.

Source is DVB, the channels show up fine with the NextPVR program but show as 4:3 when played from within XBMC.

When you're viewing video fullscreen, if you click the small film reel icon to view the Video settings, have you maybe clicked the 'View Mode' setting forcing it to 4:3?
