I currently don't have OpenELEC installed on my ATV1, but I thought the default audio settings put sound through all outputs as long as it was stereo. I saw wiki pages on the OE site for settings for individual outputs, but not two at once. Shouldn't be top hard, though, but without it in front of me, I'm not much help. Although it is ATV1 specific hardware, I'll move this to the Linux forum and see if anyone there has suggestions.
EDIT: I'll also get OE on one of my new spare USB drives (I finally have more than one that will boot he ATV1) and see if I can figure it out tomorrow, if no one beats me to it.

Did you have any luck with this ? I have the same issue. The hack from the openelec also didn't work for me.

Bummer, amplifier is not an option for me. Did you try the RCA connections at any stage ?
