What's with the blank series?

This is in quartz skin, but the same effect is present in confluence. "I" reveals no info on this mysterious series. Is this a database snafu? (BTW I am still on Eden on this machine, and DB is via mysql.)

Maybe you have a tvshow.nfo that doesn't have <title> and other metadata specified?

Because it wasn't an album. Changed it to a direct img tag for a single picture.

If you have a skin that shows additional info on items (original path), this might help. Otherwise I would delve into the database.
