I've always had this problem and I really need to resolve it. I don't think it is an XBMC issue but maybe you guys can help.

I am using an Integra receiver and a custom built windows based xbmc computer that is on HDMI input 2. My direct TV is on HDMI input 1. If I am watching a movie in XBMC then want to change inputs to watch DirectTV for a minute then change back to the XBMC input, the XBMC window has changed to a very small window at the top left corner of my screen. I have to exit out of the small XBMC window and reopen XBMC to get the full window again.

Is this a XBMC, video card or receiver issue?

Any help would be appreciated. This has been pretty aggravating.

Windows is detecting the hotplug change in the HDMI port and changing the monitor configuration. It might sound weird, but the easiest solution that others and myself have found is the HDMI pin 19 trick:
