XBMC audio sync on streamed content

I have a really strange problem with audio sync. Audio is about 1/4 of a second early on streamed content, like live TV from TVHeadend or the DR Nu addon. If I record the content to disk in TVHeadend or play other files from disk, the audio sync is perfect. Right now I watch mostly streamed content, so I keep the audio offset at delayed 275ms, but that means I have to adjust it back to zero if I watch a file.

What could be the reason? I am not letting TVHeadend do any reencoding when saving files or streaming, so the content should in theory be completely identical in the two cases.

I tried switching audio sync method and even disabling syncing to display, as well as disabling changing refresh rate to match the content. Neither made any difference at all.
