What is the best method to manage media metadata without relying on the scrapers? I am finicky about the artwork and data displayed and want to enter it myself.

You will have to use some external tool - a library manager.
Also you might consider choosing "Local information only" as scrapper for the source of your media.

XBMC handles music metadata in ID3 quite well, AAC as far as I can remember is also supported.
It doesn't do MP4 (and to my knowledge, MKV) metadata either. I've requested MP metadata reading to be added but it seems no one is really interested in it.
