1) Are you setting into full screen windowed mode, or just windowed mode? Tool bar, you mean windows tool bar? Are you using the slash key to jump between screen and windowed mode?
2) Not sure what radio button you're clicking on.

"Radio" only works if you have a PVR add-on set up and that PVR add-on sends radio channels to Kodi.

You could make a custom main menu link, but not sure which skins do this.

In Amber I do exactly that - have the "My Stations" section of the Radio add-on set as a favourite, and then link that on the main menu as a tab called "Radio".
Works a treat.
