Ashlar!
What refreshrate you are running your XBMC GUI? I ask because I noticed a strange thing. I used to run XBMC GUI at 50Hz. Running 24fps movie caused switching to 24Hz for me and playback was smooth. But running PAL movie made no refreshrate switch and wasn't smooth at all!! I struggled but had an akward idea. I switched my XBMC GUI to 60Hz (I have almost no NTSC material) and tried playing PAL and 24fps. And guess what. It seems that after switching refresh rate (at least for me) the playback is smooth. Without refreshrate change the playback stutters even if refreshrate matches the fps of the movie. So for me the key point was to change refreshrate - after change is made everything is smooth.
I was thinking if you have your GUI set to 48Hz, maybe that's causes jerkiness even tho refreshrate matches movie fps...

I was waiting for Jester to post an updated external player build. I'll try tonight.
dpassent, what you are suggesting is to keep the GUI at 60Hz allowing XBMC to switch to either 48Hz or 50Hz automatically? Is that what you are doing now? When you go back to the GUI it switches back to 60Hz?

yep... exactly... switching to another refreshrate was a key to smoothness in my case... the switch has to be made to make playback smooth.

Related question to the auto refresh thing...
I am going for a projector and a scaler, which I will hook up to my computer. Having an external scaler I don't want to have any scaling whatsoever done by the computer, so XBMC needs to output at the movie's native resolution.
I.e if it's a DVD @ 480p (NTSC) or 576p (PAL) and so on.
Is it possible to have the resolution switched automatically as well, based on content? I can run any XBMC version required (Live, Windows, Linux).
Thank you.

Settings->Appearance->Screen
Settings->Video->Player
Note that it's not autoswitching.

And check the subtitle position while you're at it because it seems to default below the screen for some resolutions.
