"Sync playback to display" and "Vertical Blank Sync" Question

When enabling the "Sync playback to display" option, should I enable or disable the option "Vertical Blank Sync" under the appearance section? The reason I ask is because when I enable "Vertical Blank Sync", the video I watch speeds up every few minutes (plays maybe 2 seconds of video in less than 1 second). I'm assuming it's trying to catch up since it got out of whack due to trying to keep in sync with the vertical blank. Enabling "Sync playback to display" doesn't seem to fix the issue for me, even though the debug log doesn't say it's not working. So I was thinking of trying enabling the "Sync playback to display" option and disabling the "Vertical Blank Sync" option. But would that even work? Cause how else would the first option make sure that all frames are delivered before a vertical blank happens?

Sync playback to display and vsync have nothing to do with each other, one is to eliminate jerks and the other is to eliminate tearing.

You should turn vsync on all the time, nobody likes tearing.
