Video card overheats under certain circumstances

HTPC Setup:
Processor: AMD Athlon 64 x2 4550e low-TDU processor
Motherboard: Unknown, think it's an ASUS that was bundled with the 4550e.
CPU cooler: Thermaltake Meorb (low profile)
RAM: 4gb DDR2
Video card: Sapphire/ATI Radeon HD4550/512mb (passively cooled)
Case: Lian Li PC-C39
Operating system: Windows 7 Professional (64 bit)

This isn't something that happens every time that I use XBMC, but it only happens when I am using XBMC, and I've narrowed it down to when it actually happens.

If I am using XBMC to watch a video file, and I fall asleep or otherwise walk away, the show will end and my computer will put itself to sleep after it is shut off.

If I wake my PC after it's been put to sleep and turn on a new video file, it will also go through the expected behavior (end movie file, go to sleep). However, if I wake up the PC into XBMC and DON'T start a new media file, and attempt to let it go to sleep, it will not go to sleep, overheat my GPU while it should be idle, and freeze up my system.

Has anyone else noticed this sort of behavior?
